Effective April 1, 2025 Local 68 will be switching to Horizon Advantage EPO.

This is an upgrade from our old OMNIA plan and has a larger network.

Most OMNIA providers DO participate in Advantage EPO.

This does NOT affect existing Rx, Vision, or Dental coverage. No changes there.

You should have received your new cards in the mail already. If not, you can access them online from the Horizon Website after April 1 or request them from our Fund Office.

For out-of-state care, we still have Blue Card PPO, so just make sure that the provider participates with the Blue Cross PPO of that state, and it counts as in-network.
